To schedule an appointment with our practice, please have your primary care physician or referring medical doctor submit a referral to us via facsimile at 1-855-955-3905. Upon receipt of the referral, we will conduct a thorough review and communicate our decision regarding the referral request to your physician. Once the referral has been approved, a representative from our office will contact you directly to arrange an appointment at a convenient time.

Please note that a referral from your physician is required prior to scheduling an appointment with our practice. If you have any questions regarding the referral process, please contact our office and we will be happy to assist you.
