We have a modern electronic medical records that allows you to communicate with your physician and access your results via a secure patient portal. You will be asked to fill out a short survey to update all your demographics including address, contact information, etc.
ABOUT YOUR VISIT
Preparation for your appointment
Before your initial appointment, you will be asked to fill an intake questionnaire that will be sent to your email and/or cell phone. Please make sure to fill this out prior to your initial consultation. In addition, depending on the reason of your visit, you may be receiving additional questionnaires that will be sent to you via email.
